Appointment Instructions
You have scheduled an appointment in the Fitomics Fitness Lab. Please review the following preparation guidelines.
How to Prepare
Please arrive well-hydrated and 3–5 hours fasted from food and highly caffeinated or calorie-containing beverages. Please drink plenty of water or calorie-free fluid before your appointment. Avoid any strenuous exercise or activity within 12 hours of your appointment time.
What to Wear
Men
If you are completing the 3-C Advanced Body Composition Assessment, please wear compression underwear or compression shorts under your clothes for the 3D scan. If you are completing the Exercise Prescription Assessment, the Comprehensive Exam, or the Active Metabolic Rate Assessment, please come dressed in athletic wear.
Women
If you are completing the 3-C Advanced Body Composition Assessment, please wear compression bottoms and a sports bra (a 2-piece swimsuit or regular underwear is also OK) for the 3D scan. Please come with your hair pulled back in a bun. Please refrain from wearing face makeup. If you are completing the Exercise Prescription Assessment, the Comprehensive Exam, or the Active Metabolic Rate Assessment, please come dressed in athletic wear.
By scheduling and participating in any Fitomics service (“Services”), you (“Participant”) acknowledge that you have read, understand, and agree to the following terms:
1. Services
Services may include exercise testing, personal training, nutrition coaching, and collection/use of biological samples.
2. Risks
Participation involves inherent risks (e.g., strains, cardiovascular events, illness, or in rare cases, serious injury or death).
3. Informed Consent
You confirm that Services have been explained, all questions answered, and you agree to follow all instructions and report any discomfort immediately.
4. Medical Clearance
You affirm you have obtained any necessary medical clearance and will inform Fitomics of any health changes prior to each session.
5. Data Privacy
Fitomics will comply with applicable data‐privacy laws. Personal data and samples will be used solely to deliver Services and not shared without your written consent.
6. Liability Waiver
You waive and release Fitomics and its agents from any claims arising from participation in the Services.
7. Indemnification
You agree to indemnify and hold harmless Fitomics from any claims arising out of your negligent or intentional acts.
8. Governing Law
This agreement is governed by the laws of Alabama or Georgia, as you choose when scheduling.
9. Severability
If any provision is invalid, the remaining terms remain in effect.
10. Consent
By scheduling my appointment, I acknowledge that my participation in the scheduled event serves as my signature and full acknowledgment of these terms.
